Lines and wrinkles are a natural, and unstoppable, part of the ageing process. While they are inevitable, they can be both prevented, and reduced, by using botulinum toxin type A. It is still the most powerful anti-ageing tool in aesthetic medicine to date. It works by temporarily relaxing the facial muscles. Q: I’m considering Anti-wrinkle injections but have never had it before. Will I see immediate effects?
A: When you have botulinum toxin type A, it takes between 2 – 14 days for the results to fully develop. This is because the chemical reaction necessary for botulinum toxin type A to take effect on the nerve endings is not immediate and develops over a few days.
Q: Is Botulinum toxin type A Permanent?
A: No, botulinum toxin type A is not permanent. It works by blocking certain nerves in your body which relaxes those muscles. Immediately after having the treatment, your body will begin repairing those nerves, and in 4-6 months they will be back to normal again.
Q: Are Dermal Fillers & Botulinum toxin type A the same thing and work in the same way?
A: Fillers and botulinum toxin type A are two different treatments that work in different ways. Botulinum toxin type A is a drug that relaxes the muscles, causing wrinkles or lines to soften. Fillers, on the other hand, are volumizers and can be used to give a non-surgical facelift. They are used to plump a sagging face and require a deeper injection underneath the skin.
Q: What side effects are there to Botulinum toxin type A?
A: Botulinum toxin type A is as safe as any drug when administered in very small amounts by trained aesthetic doctors, nurses and dentists. The most common side effects experienced are headaches, bruising or slight swelling, which result from the injection itself rather than the drug. Side effects can be experienced by those who are allergic to the substances used to conserve the drug, which can affect around 1 in 5000 people.

Q: Can anyone have dermal filler treatment?
A: While most people can benefit from this treatment, certain anticoagulation medications and conditions preclude its use. If you are pregnant or breastfeeding, you would be advised to delay treatment until breastfeeding has finished, this is merely a precaution. Keloidal tendency, active skin infection in the area of injection (cold sores, spots, ulcers, acne) are also contraindications.
Dr Michele Engel will ask some questions to ensure that dermal fillers are appropriate for you or not.
Q: What fillers best treat areas and concerns?
A: Fillers will help to restore our skin and facial contours to their former youthful appearance by plumping and firming the skin and filling out wrinkles or re-volumising the face. It’s widely used to address ageing hands and medium to deep lines and wrinkles such as nasolabial folds (nose to mouth lines), fill scars, contour or re-volumise the cheeks and chin or to augment or reshape lips and nose.
Q: How long do fillers last and can you get permanent ones?
A: The length of time that dermal fillers last depends on the type of filler used and the area injected. On average, you would expect the results of a Hyaluronic Acid dermal filler to last around 6-12 months. Thicker dermal fillers tend to last longer than those used for finer lines. Permanent fillers are available but not widely used in the UK. Cosmetica London only uses resorbable fillers (non-permanent).
Q: What substance are fillers made from?
A: The most leading dermal fillers used in the UK are hyaluronic acid based products. Hyaluronic acid is a natural component of our skin which can be synthesised. Other non-permanent fillers to be developed include ones using polylactic acid and calcium hydroxylapatite.
Q: Should I expect any side effects from the treatment?
A: Like any injectable treatment, there is the risk of bruising, swelling, bleeding and redness in the injection area. You may also get headaches and respiratory infection.

The TCA peel ( Trichloroacetic Acid ) is a medium peel and is a very popular treatment for skin of all colours and imperfections. TCA peels can vary in strength from 10% to 50% depending on the skin tone, imperfections, wrinkles that are in need of improving.
TCA peels at higher concentrations produce deeper peeling, as does the number of applications of TCA solution applied to a given area.
TCA peels can improve wrinkles of the skin by improving the collagen and elastin fibers of the skin, strengthening and repairing them. TCA can also improve the appearance of acne scars, fine lines around the eyes and mouth, discolouration associated with the sun, hormones, and ageing.
TCA peels will result in the skin having a red appearance for about 7-10 days and require absolute sun protection during this time.
Q: What skin conditions/concerns are best treated by a chemical peel?
A: Almost everyone can benefit from a chemical peel. Peels can improve the look of your skin dramatically and can leave you looking younger and healthier. It’s great if you suffer from any of the following – Dry skin, Oily skin, Acne, Acne scarring, Lines and wrinkles, Pigmentation, Sun damage, Dull or lifeless skin.
Q: I’m thinking of having a peel, how do I prepare my skin prior to it?
A: You should avoid use of the following products and procedures on the area to be peeled for 5-7 days prior to the peel: Laser hair removal, Waxing, Microdermabrasion, Depilatory creams and Electrolysis.
There are certain products that may increase skin sensitivity and/or cause a stronger reaction to the peel if used up to the day of the peel. Avoid use of the following products for 3 days prior to the peel: Topical retinoids and tretinoin (such as Retin-A®, Renova®, Differin®, Tazorac®, and Refissa®)
Other topical acids or exfoliants that may be drying or irritating to the skin (such as alpha hydroxy acids [AHAs], beta hydroxy acids [BHAs], exfoliating masks, retinols, hydroquinone, and benzoylperoxide acne products)
Q: What types of peels are there?
A: Superficial/Light Peels – They only remove the top layer of the skin, known as the epidermis, and can give your skin and instantly brighter look as well as a smoother, more even texture. They can be used to minimise fine lines and wrinkles, sun damage, mild acne scarring, age spots and dry or flaking skin. They are usually made from alpha hydroxy or beta hydroxy acids, known as AHAs and BHAs and include glycolic acid, lactic acid and salicylic acid.
Medium Depth Peels – Medium depth peels give more dramatic results than superficial peels. Usually derived from TCA (trichloroacetic acid), they penetrate deeper in to the skin and can be used to treat sun damage, pigmentation and wrinkles.
Deep Peels – Deep peels are the strongest type of chemical peel available and can achieve amazing results for sun damage, scarring and deep lines and wrinkles. They usually use carbolic acid (Phenol peels) or high strength TCA to penetrate the deeper layers, or dermal layers, of the skin.
Q: What should I expect after a peel?
A: Superficial/Light Peels – You may get a little bit of pinkness and some mild skin peeling for a couple of days but will be able to resume normal activities straight away.
Medium Depth Peels – There is a longer recovery period, usually a week to 10 days, although your skin can be pink for a few months. You will also get more peeling and your face might be quite sore and swollen and feel tight after treatment.
Deep Peels – Recovery from a deep peel can take a couple of weeks. You are likely to experience pain and discomfort after the treatment, as your face will be tender and swollen and feel almost sun burnt. You should take recommended painkillers to manage this. You will get quite severe peeling, which can look fairly horrific and be incredibly itchy. This is usually at its worst a couple of days after treatment. You must avoid scratching or picking any scabs or bits of peeling skin as this can result in scarring. It can take up to two weeks before you start to look normal again. Redness can take even longer to disappear and may last three or four months.

Q: What brands do you use at Cosmetica London?
A: We use two brands, the original, Genuine Dermaroller® and Medik8.
Q: I’ve read there are different size needles and I can do the treatment at home?
A: There are a variety of needle sizes which range from the tiny 0.2mm to the larger 3mm. You can purchase the Dermaroller for home use at Cosmetica London with needles sizes 0.3mm and 0.5mm. For medical treatments at the clinic, Dr Mica Engel will use either 1.5mm or 2mm depending on what skin condition you have.
Q: Do Dermarollers work?
A: Dermarollers are supported by clinical studies to be effective in non-surgical and non-ablative treatment of a variety of skin conditions. Studies have also shown that Dermarollers are often a better treatment than microdermabrasion, chemical peels, and laser resurfacing. They are proven to be just as effective in their ability to stimulate collagen and elastin production. Regular use of Dermaroller can thicken skin, smooth scars of all sizes, and erase wrinkles.
Q: Are there any contraindications?
A: Yes, active infection on the area to be treated and history of abnormal scarring e.g. Keloid. Loose skin is also a contraindication (if treating stretch marks).
Microsclerotherapy is a safe and effective technique used to treat spider veins or thread veins of the legs. It is a relatively straightforward procedure which involves injecting a special solution, into the veins which destroy them and causes them to disappear over time.
Generally, four treatments, spaced several months apart, will achieve the best results.
**Note: The use of compression stockings is recommended for two weeks after every session.**
Q: What’s the difference between spider and varicose veins and can both be treated with Sclerotherapy?
A: Spider veins are smaller, red, purple, and blue vessels that twist and turn. Spider veins are easily visible through the skin. They are most often seen on the legs and face and are completely harmless.
Varicose veins (varicosities) are bulging, abnormally enlarged veins near the surface of the skin, commonly found in the legs or pelvis area. The failure of these veins to circulate blood properly causes blood to pool within the vein. Each vein has a series of valves, which are designed to prevent blood from flowing backwards. When these malfunction they allow blood to collect creating pressure inside the vein and form protruding, unsightly veins that are often accompanied by discomfort or pain.
We are only able to treat spider veins at Cosmetica London. Varicose vein removal requires surgery which your doctor can help you with.
Q: What causes these veins?
A: There are some factors that can cause a person to suffer from varicose and spider veins. These include:
- Occupations that involve a lot of standing, such as nurses, hair stylists, teachers, and factory workers
- Hormonal influences during pregnancy, puberty, and menopause
- The use of birth control pills
- Postmenopausal hormonal replacement
- A history of blood clots
- Conditions that cause increased pressure in the abdomen including tumours, constipation and externally tight worn under garments
Q: Is there any harm in removing spider veins?
A: Spider veins have no function and removing does no harm. The only reason they are removed is because of their unsightly cosmetic appearance.
Q: When is the best time to have Sclerotherapy?
A: Although you can have Sclerotherapy all year round, Dr Mica Engel advises to have it done in the cooler months as wearing the compression stockings can make you feel hotter and constricted in summer months. You also can’t have direct sunlight on the treated area for four weeks after the treatment.

Q: I’ve read Cryotherapy can be used to treat a number of conditions, what do you use it for at Cosmetica London?
A: That’s correct, it can be used for numerous health issues from serious illnesses like cancer to eye problems, sports recovery, headaches to sexual conditions and superficial benign skin lesions like skin tags, warts, moles, freckles and verrucas. We only use it for the removal of skin conditions.
Q: Are there any side effects to this treatment?
A: Discomfort can occur both at the time of treatment and for a variable time thereafter. Taking painkillers like paracetamol for the first 24 hours should help that.
Swelling and redness is a normal and immediate response and usually settles after two to three days. For a short while the treated area may ooze a little.
Cryotherapy close to the eyes may induce prominent puffiness of the lower eyelids which settles within days.
Blistering is also a common consequence of cryotherapy and blisters settle after a few days as the scab forms. Some people blister more easily than others and does not necessarily mean that the skin has been frozen too much. Occasionally the blisters may become filled with blood; this is harmless and only if a blister was very uncomfortable would it be necessary to puncture it, using a sterile needle.
Infection – uncommonly, infection can occur, resulting in increased pain and the formation of pus.
Q: Will my skin lesion disappear after one treatment?
A: After the healing process, smaller lesions should disappear after just one treatment. Larger and thicker ones may well take 2-3 treatments.
Skin tag removal is now easy and painless if done correctly with Diathermy. Skin tags themselves are benign and can arise spontaneously. At any age, they tend to appear as people get older. On the face, eyelids, neck, armpits, chest, breast, around the groin areas, skin tags may appear. They come with the colour of flesh or a slightly darker pigmentation.
Diathermy treatment is extremely quick and effective as it cauterises the skin tag at the base and seals the wound.
